For example Telephone number: Area Code (text box) telephone number (text box)
How do you wish to vote? For ( check box) Against (check box) abstain (check box)
Departure date (text box) Return date (text box)
You can achieve the desired layout by doing the following
The phone field already has a code and number aligned by default
Add check-box items then shrink them to occupy half the column
Next set the labels to align to the left
Next add a text to act as a buffer between the checkbox then add the from and to date fields and set them to be shrunk also (date fields are accessible under the quick tools section)
Finally ad the following CSS using the designer to align the labels even better
width : auto !important;
