I have been creating a feedback from which was working perfectly - but when I completed my final test send, the submit button (when clicked) that the 'form is currently unavailable'?
Why is this happening and how do I fix this urgently?

Upon checking, I noticed that you have set a limit on submission in your form. See screenshot below.
You can remove the limit or replace with number of limit you want. See guide: How to Limit Submissions in a Form.
