I want to embed a form to my html file as below link and html

However, when I view this page in iphone(safari), the form will be not normal.
Can you help me? Actually, I want use this form in all mobile devices.
Now I will attached some photos which is not embedded in html file, this is show as well.
Instead of using the script embedding method, kindly use the iframe embedding method
