I have two forms that are virtually identical. One of these forms (the order form) has conditional areas that are working fine. The other form (the quote form) has identical conditions set and does not work.
Please help! I need to publish these soon! The link to both forms is here as well as in the 'URL of the web page that has this form' below.

Have you tried clicking and conditions and saving it again?
Here's how I did mine when I cloned your form:
It worked after making the changes. Try clicking and saving the existing conditions, see if that will make the form work.

Funny, cuz after trying to get this to work literally all day, I finally gave up and just had those areas always be visible on my form (not by choice, just by the fact that I've spent enough time on this). But when I saw your video, I figured I'd reset all of the conditions and try again, exactly as you showed.
It worked very nicely on my preview as well. It still doesn't work on my website unfortunately. Not sure if you saw my previous posts but this exact same form (albeit my order form) with the same conditions is working like a charm (on my website). FYI, I'm working w/ both of these forms on the dashboard of my website, not publishing until all bugs are worked out.
This has been a very frustrating day!

Apologies for the inconvenience, I understand the frustration when something is not working and you tried to troubleshoot it all day.
We'll do our best to assist you on this matter.
I'm checking the forms again and I see that they are under the account username ntrombley. Is this also your account?
I'm checking the form titled "SS Quote Form" and there are no more conditions setup there. Perhaps you have deleted them? Same goes with the form "SSU Order Form".
May I also know what website builder you are using, is it Wordpress? Could you try editing the form in the form builder directly here in www.jotform.com. After that, if you want to embed it in your website, you can follow this guide: https://www.jotform.com/help/324-A-Guide-To-Forms-For-WordPress-JotForm-Users
See if directly updating the form using the form builder here works.
Also, if the forms are embedded on the same page and you are using the default embed script code, that may be causing conflicts between the form's resources, because they load the same JS libraries. If that's the case, you might need to embed the forms using the iFrame embed code.
Please do let us know if my presumptions are correct, if not, we might need more information on how you are setting up your form and website.
