The Credit card section of my form sometimes does not display, why does this keep happening?

I presume you are referring to the To-Go Lunch form, I have tested it on my browser and the fields are showing when selecting the card option.

Have you embedded it on your site? if so kindly use the iframe embedding approach to prevent script conflicts.
