Como crear un formulario para descargas?

Buenas tardes, tengo un archivo descargable para mis empleados en un sitio web, y deseo saber cada cuando lo descargan para llevar un control. ya que se supone lo deben descargar cada dia.

Me gustaria saber si es posible crear un formulario en el que el empleado introduzca su nombre y descargue el archivo, al momento se genere el registro del momento en que lo descargó y asi poder llevar el control que necesito.

Though it would not be possible to be 100% sure the employee downloaded the file, you could create a form that asks for the employees name, asks them to download a file, then asks them to confirm they downloaded the file.  I setup a quick test form to show you what I mean:

This would at least give you a record of who filled out the form and saw the download link.
