I have posted a new HTML page on my website with the form embedded as a script as you suggested. On my end, the prepopulating works fine..but for some other users that are testing..the forms opens as normal but the prepopulating fails. Here are the links:

I checked your form http://www.jotform.ca/form/52336975311254 and I think the issue has more to do with your account/username  kodiak  being already at Overlimit Status due to exceeded submission limits on SSL.

You will need to resolve first the temporarily disabled account due to Overlimit Status.

You can either wait for next month when the auto reset occurs to clear the subission count.

Or upgrade to the higher plan to have expanded limits. Upgrading the subscription will immediately take effect and will reinstate the account and forms immediately also.

You can upgrade using the upgrade button in the JotForm Pricing page.

Hope this help. Let us know if you need further assistance.
