My Form has been disabled

    Hello, I have a form that I am creating were my customers can sign up to a service we provide, but charge later for. When I choose to preview it it shows my form has been disabled. 

    Can you help me? Thanks!!

    I have checked your account and your account is autosuspended by the phishing filter because of the suspected phishing form you have: http://www.jotform.us/form/51907637266160

    Inside of mentioned form you have a password field and that is against our Terms of use

    Phishing - You agree that Company may terminate your JotForm Service immediately If a form is created or JotForm is found to be used to: Obtain credit cards sensitive information, get social security numbers, show content with child pornography. Or if a form was made to steal user login credentials. All these activities are considered as Phishing and any account along with the guilty forms will be suspended immediately. 

    You need to remove Password field form your form and only then we can activate your account.
