How to add form in my website?

I have a scholarship memorial fund website in the name of my son and we have a golf outing each year to raise money.  I would like the golfers to complete a golf registration form online (accessed via th following website; www.WoodysFund.org).

I have created a form on JotForm and now would like to test it on my website but not sure what the next step is.  

Could someone at JotForm call me at (517)420-0208 - - or could you provide me a support number that I could call ?
