Request a Theme Inquiry

There is a current JotForm theme that I would like to modify and use as a 'permanent' theme for our business.  I submitted information on the 'Request A Theme' page, but have not received a response.  My request is to use the 'Big Blue' theme, add our logo to the top, change the font and resize the submit button.
