To my understanding you are experiencing a strike-through problem with a line going through your sub products when attempting to follow our guide?
I took a look at your account but found that you have several forms. Please provide us with a link to the form you are having this trouble with so that we can take a look and help guide you in the right direction.

I am talking about the form labeled "Order Form". Also I have noticed that on my website Eventflyr.com the form doesn't show up whenever i access the homepage from within the site. So if i click the portfolio, then return to the home page the form disappears. It only loads when the site load for the first time. How can I fix this also? This poses a major issue because if my customers want to view my portfolio before making an order the order form completely disappears.

It is possible that the form was having scirpt conflict issue on your website because it was embedded using the default script code embed.
Using the iframe embed code should prevent any script conflict issue form happening.
Make sure you clean/clear first any existing previous script embed code of the form, before you add its iframe embed code.
As for the Sub-Products option, you will need to add first the Quantity option before you can create the Sub-Products
Hope this help. Let us know if the issue persist
