Unexplainable bullets on the left of the form

Hi,

I cannot seem to figure out why my forms come out with bullet points on the side of each item.  I have tried creating a whole new form and I still get the bullets on the sides.  Any idea of how to get rid of them?  

I am attaching a screenshot. I get the same problem on Firefox and Chrome.   I have also tried the put the form on a blank page without any sheets linked and still get the exact same problem.  Originally the form didn't have this problem, it just came up after a month of it being created.

Any help would be greatly appreciated.

It seems that CSS file is not loading on your local web page.

We include form resources without protocol on the source code, so this might be an issue in your case. You can try either loading your test page using a local web server, or adding the protocol to the resources in order to fix the issue.

Example:

//d2g9qbzl5h49rh.cloudfront.net/static/prototype.forms.js

changed to

http://d2g9qbzl5h49rh.cloudfront.net/static/prototype.forms.js
