However the information does not get to my Infusion Soft Account. I am looking at the form HPT contact form.

Is it all the field values that are not passed on your infusionsoft CRM or just some fields?
May I suggest to try re-integrating your form to infusionsoft first and see if that makes any difference. If not, try to make a clone of your form and try this minor changes. Use the fields from the quick tools without changing its default config.
Example for the email field, try not to enable email confirmation, and for the phone field, take off the input mask. I'm not certainly sure if this helps but I have suspicion that this 3rd party integration does not support customized fields and might work with default fields.
