Do I need to embed the form to our website for people to fill up?

I own a print and sign shop and need to create a custom form that anyone whether a customer comes in

or calls on the phone can open our blank form fill in the necessary info and email to the billing and art department.

Also if possible the sales  people can open it up to fillout with a customer if they are at there place??

Can this be done

Or is this form has to be online on our website?  Its basically for my employees to fill-out?
