How to get the Form URL link

Hi, I'm trying to link to one of your forms from my personal webpage and it is not working.

The questionnaire link is working, but not the contract link. See URL.

the Form link to the Sign Contract Here is wrong and invalid. It was the reason the form is not loading.

The link to the Questionnaire is correct so it loads the form http://form.jotform.us/form/52355591668163

I suggest you correct the link URL to the Contract Form http://www.jotform.us/form/52365968628168 and it will load properly.
