Perhaps there's a problem when you embedded the form in your iWeb?
You can try following this guide on how to embed your forms to iWeb: https://www.jotform.com/help/12-Adding-Form-to-iWeb.
If that doesn't work, you can try using the iFrame embed code instead.

I'm glad to hear that the iFrame worked.
Regarding the empty embed codes, I checked it in my end and I see what you mean, it seems like there's a problem with that option. I'll report that problem to our back end team.
