Widget Terms and conditions check box

When I add a Terms and Conditions in a form, it doesn't appear check box. What am I doing wrong?

Now I have my forms closed? Why??????

Sometimes are open, sometimes not. In any case I can't see the check box

The code in the above pages seem to be still pointing to the retired formcentral forms, kindly update them to point to the form created on jotform by embedding using the iframe method
