How can we remove "Sample logo" at the top of the form?

My form has an image that says "Sample Logo" at the top, and I can't figure out how to get it off. It appeared after I hoped the Designer. Can you help get it off?

Yes, I was able to remove it. As soon as I submitted the request, a page opened that had the solution.
