Form requires actual browser for it to work properly. Embedding the form on email will stop the form from working properly since emails are very strict with html codes. I'm afraid all the form scripts will most likely be deleted once you embed it on email. I would suggest to use the form URL instead and place it on the email body so the form will load on an actual browser once it is clicked.
