Autoresponder email not working

Please help me with this. My email autoresponder is not going out. I've put in conditions so that when met it's supposed to go out and it doesn't. I've removed the condition so that it's just an autoresponder and it doesn't go out. I have an email called [copy] and it seems to go out, but not the main autoresponder.

Seems like you are using a custom sender address, instead, please use this workaround:

Also, the conditions seems to be unnecessary, if the fields are set up as required, then there is no need for such condition, as it will ways be filled.
