The reason why that happened is that you built a form to catch credit card sensitive information in plain text fields.
Card number, expiration date, security codes and card holder name are considered critical information. The only services allowed to collect such data are those that are DSS/PCI certified.
In light of this, you can open an new account and make a new form, but it is a must to integrate your form with any of our Payment integrations.
Otherwise, the system will suspend your account in no time again.
