Our form titled: August Questionnaire (http://www.jotform.us/form/52283851750154) does not appear to be displaying all conditions accurately. There are two questions that show all conditional questions immediately upon opening the survey, however I've created conditions that would hide them until specific conditions would reveal them... can you please take a look?

I am currently checking your form. Please give me more time to check this. I will get back to you as soon as I get the fix for this issue.

As per checking, I noticed that field are shown due to some of your conditions are conflicting or incorrect. For the SEASONAL RECIPES section:
There are fields that should be hidden upon load but shown due to its condition. The conditions set for this field is conflicting. 4 and 5 conditions are conflicting (refer to the screenshot below) and also based on the condition the default value for Will you be featuring any of the following seasonal ingredients in the next three months? field is empty. Empty value is not equal to "none of the above" or "nothing new since last communication" so the field is shown by default.
To fix this you will need to have this condition. (Remove the #4 and #5 condition and replace it with the condition below.)
For UPCOMING FALL/ SEASONAL EVENTS section:
As per checking the condition for this section, it seems that the fields that are being shown/hidden are wrong. The field that is being show/hidden are in the UPCOMING FALL/ SEASONAL PROMOTIONS section. See screenshot below.
To fix this, just change/edit the fields that are being shown or hidden to the correct field.
You can check this cloned form that I used for testing for a closer look on the setup of the form - http://www.jotformpro.com/form/52389039857975.
