Can you try to add a Mobile Responsive Widget to your form and see if it will work?
If the issue still persists, can you provide to us the link or name of the form in question so we can further investigate the issue? If you have embedded your form in your website, please also provide to us the exact URL where you have embedded the form?

Are you unable to see the tab for your form when using the Facebook application on your iPhone?
If that is the case, tabs on Facebook pages are not enabled in the mobile website. User would have to use the desktop version of Facebook to access the tabs and your form.
If you would like mobile users to fill out your form, you can provide them with the direct link to your form on your Facebook page.
