I am not getting any emails telling me there was a form submitted. I used to get emails back when I first started here but something has changed and I don't get them anymore. How do I correct this so I am notified of a new submission?

Your first notifier doesn't have a recipient. Please check that by following the steps from this guide: http://www.jotform.com/help/41-Finding-out-E-mail-Address-used-for-submission-notifications.
Your second notifier is correctly configured, but you are using a custom email address in the settings. I'm not saying this won't work, but I would suggest checking this with your hosting provider if they allow this in your current plan and make sure that the details are correct. So far, I can only see your tests in our logs. Also, make sure to use the proper email capitalization.
Your third notifier is also correctly configured. You are using our default noreply@jotform.com address as the sender.
This guide should help you more about this kind of problem in the form: http://www.jotform.com/help/208-How-to-setup-email-alerts-to-prevent-email-bouncing-related-issues. I would suggest doing our recommendation such as the whitelisting part.
Lastly, you don't need to create multiple notifiers. You can just use one. Here's how you can delete the others: http://www.jotform.com/help/132-How-to-Delete-Notifications-Autoresponder.
