My contact us page is asking for authentication when my form is embedded there

    my contact us page for Blind Guy is asking for FTP username & Password when code is placed into webpage. Never had this promblem before

    I cloned the form from PSI Window coverings which works perfect!

    We don't put an authentications to our forms so I'm sure that is related to your website. But this is the page: http://www.blindguy.net/contact-us/index.html and I don't see the form here.

    If you want to embed it, I would suggest using our iframe method: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code.

    I took the form down as it was not working.

    There is an issue with two links on your source code.
