I would like to apologize for the inconvenience. I've checked the conditions on your account. I've noticed that when the conditions are met, it will send the notification setup on your form to the corresponding email address. I saw that most of the conditions are sending notifications to multiple email addresses which are separated by a semi-colon and space. That might work, but it is not recommended. I would suggest you try to separate the email addresses using a comma and space instead and see if it's going to work properly. Thank you.
