Can I use my own personal domain instead of having jotform appear in the URL?

Hi,
Thank you for providing us with more details.
The user guide -How-to-use-Custom-Domains-with-Jotform was referring to the CPanel option in your website.
on Step#1
To use your own domains in Jotform, add a DNS record:
Add a CNAME record which points to custom.jotform2.com with your subdomain. Here is what it will look like in the DNS:
forms.my-company-domain.com CNAME custom.jotform2.com
so, you need to add first a CNAME record which you will use later as the URL
Example: http://forms.my-company-domain.com/form/1234567890
The URL you are pointing on your screenshot is NOT the custom domain URL.. that is the custom URL Name of the form (if you want to use custom URL Name)
