My images don't show up when sending a link to the form. can you help?

    I have an image set up at the top of my form.   There is a blank square that appears in the place of the image.

     

    I has not always done this.

     

    Can you help me fix this issue?

    I can see the image on my end so it should be fine.

    Please clear your browser's cache and check the form again. This might help.
