I'd like a form that requires payment via PayPal to accuate.  Three questions only - Name - email address - Other Name

The "Other Name" column gets posted to the same website page in a rolling list.

I already own the domain and have hosting

I would like the page like this

    Logo

    Hi Dan,

The form is very simple. You can just create it in our form builder, but I would suggest creating an account first before doing that so you have the complete control of the form and can get back to it later for changes.

As for the "other name" field usage, I'm not sure if I understand that part. Can you explain that further?

    Thanks

I can create the form no problems.

If there are four questions -

    What is your name?

    What is your email address?

    *What is someones name?

    *What is someones email?

    Pay

The * questions are then posted to a list on a website.

If you want to post the submissions to a custom page, that will require you to connect the form to your database and display it using PHP queries.

Here's our guide on how you can send your form submissions to your MySQL database:

    http://www.jotform.com/help/126-How-to-send-Submissions-to-Your-MySQL-Database-Using-PHP

If you want a simple way to achieve this, you can create the listing using the following methods:

    a. Create a list using our submissions view app: http://apps.jotform.com/app/submission_view. You can embed it to your website. Or...

    b. Create a table report: http://www.jotform.com/help/105-How-to-create-a-Grid-Listing-report. You can also embed the report to your website.

Let us know if you need further assistance regarding this matter.

    Thanks
