I'd like a form that requires payment via PayPal to accuate. Three questions only - Name - email address - Other Name
The "Other Name" column gets posted to the same website page in a rolling list.
I already own the domain and have hosting

Guys - we use the same OneUp Wordpress template for our websites. Only you're still using the bundled images and I got my own.
I really need a simple form done asap please. Will pay

The form is very simple. You can just create it in our form builder, but I would suggest creating an account first before doing that so you have the complete control of the form and can get back to it later for changes.
As for the "other name" field usage, I'm not sure if I understand that part. Can you explain that further?

I can create the form no problems.
If there are four questions -
What is your name?
What is your email address?
*What is someones name?
*What is someones email?
The * questions are then posted to a list on a website.

If you want to post the submissions to a custom page, that will require you to connect the form to your database and display it using PHP queries.
