To my understanding you are having trouble embedding your form on your website and that the iweb code you are attempting to use isn't working, correct?

Could you please provide us with a link to the site that you are trying to embed it on so that we can investigate this issue furthermore for you?

I can't send you a link to the site as it does not appear on the iweb app when I click Apply

Have you tried the steps provided in this guide - Adding Form to iWeb? Can you provide us a screenshot of what happen when you try to add your form on your site? Also, please provide to us the link or name of the form in question.

From what I can see so far I noticed the script is working on my end since I can see your form displayed normally when I tested this to double-check it. However, I am thinking perhaps there's a conflict with it and iWeb on your end possibly.

If I am not mistaken I believe you are doing this from Mac since you are using iWeb, correct?

Since the iWeb Code isn't working for you I would alternatively suggest trying the iFrame Code instead to see if that will help to resolve the issue since it seems like there might be a conflict between iWeb and the current iWeb Script.

If that doesn't help then may we know what OS and Version you have?

Also, what version of iWeb are you using?

I tried iFrame and the attached images show the result

At least there was something there but the colour of the page I made on jot form was not revealed see attached screen shots

My iWeb version is 3.0.4
