    Hi,

    I checked your account and you currently do not have any submissions.  You are no where near your quota.  It looks like you may have upgraded your account, rather than the account of the person sharing the form with you.  Since the form is in the account "vpicu".  That is the account that would need to be upgraded.

    If you would like, we can refund your most recent subscription payment, since it is likely the incorrect account that was upgraded.

    You could then upgrade the "vpicu" account to restore your forms.
