We would like to apologize for the inconvenience this have caused you. This issue is related to Google Spreadsheet Integration and it's causing submissions to fail.
While we are looking into this, please consider re-integrating the form with the Google Spreadsheet as this may solve the issue. Existing submissions will just be imported to Google Spreadsheet upon reintegration.
