Why isn't my form keeping the edited design changes that I make even after I save it in design mode?

Hello, twice I edited my jotform, color and look, saved it and embedded it in my website. Two days later I come back to the form in Jotform and all my form design is lost. What am I not doing correctly? See Screenshot attached to see how it was after editing and see URL to see what it looks like now. Thank you.

Our apologies. I'm not sure what happens, maybe the changes does not stick or it has inadvertently reverted after you made few changes on your form which appears to be on the logs. Anyway, you can find and restore your last changes on the revisions history. http://www.jotform.com/help/294-How-to-View-Form-Revision-History. If we can be of further help, please let us know.
