Field no Displayed

    Good morning,

     

    The lastas two fields in the form has not been displayed. I have tried several alternatives to fix it but without success.

    Can you please let us know which fields are not being displayed? I have cloned your form into my account for testing, and all fields seem to be displayed fine - or according to their Conditions.

    I can see that you use Conditional Logic to hide some fields, are you by any chance talking about these fields?

    For example, your fields on page 5, Quantos sinistros ocorreram na apólice atual? and Classe de Bônus, are conditionally hidden when Tipo de Seguro is equal to Seguro Novo:
