I believe that this is the form in question, as it is the one last modified:

    http://www.jotformpro.com/form/52217855303957

I have cloned it into my account, and created a PDF form out of it without any issues.

As I couldn't encounter any issues with creating a PDF form out of it, I would advise you to please try clearing your browser cache, and then try using the Create PDF Form option again:

The PDF form created in my test looks as in the following image:

Please let us know if you experience any further problems even after clearing your cache and trying again. Thank you.

I am getting an error that states that internet explorer will not display the webpage.

Are you unable to open JotForm webpage inside of Internet Explorer or Fillable PDF form?

If you are unable to open JotForm webpage I would suggest you to clear your browser cache or use different browser. I was not able to replicate that issue.

Regarding Fillable PDF - you have to use Adobe Acrobat Reader or Foxit to be able to fill and submit your fillable PDF form.

Hope this will help. Let us know if you need further assistance.
