The date is incorrect on my form and not current.

I can see that your form, when accessed over its direct link, is showing correct date:

It looks like you are trying to use the Source Code version of your form. The page where you have posted your form's source code is unfortunately not a valid HTML page.

Please use a valid HTML page for your website, and I would also advise you to please use our iFrame embed method instead to embed your form into your website.
