I have this form below that I made in photoshop that I would like my form to look like.  I put it onto my website and tried to upload via the webpage, but it didn't enter anything correctly.  Is there a way to add this form into Jotform?  Thanks

To my understanding you mean that you wish to create a form similar to the one you have made a mockup of in Adobe Photoshop.

Currently, that is not possible as we do not have any sort of plugin that could read PSD Files if that is what you are trying to import as a form.

We have an Import Tool but it is more intended for PDFs with FormCentral Forms users before it shut down so I can't guarantee that it would work if converted including the fact that pdf's are limited.

However, I can verify that it is possible to create something fairly similar to that but your best bet to do that would be to create a form from scratch in our formbuilder then you can stylize it further using our Form Designer to make it look fairly similar to the one you have in Photoshop.

You may even want to take a gander at our Themes Gallery to see what is possible since all those were created in the same way using a form with fields from scratch and our Form Designer including advanced styling with the use of CSS.
