having trouble with submissions imports

I am trying to do a bulk import of submissions from a test form into a production form.

I have tried, but can't seem to get the date fields correct.  I have formatted my date fields as text to match the template, but when imported, it does not look right.  I am attaching the file from which I am attempting to upload.  A related question - is there a way to copy the submissions directly from form to form?
