Hi.
Is it possible to have more than one Form Collapse Bar on one form? I could not get it to work.
In the "Voluntary Long-term Disability Insurance" section (towards the end of the form) I have successfully inserted a Collapse Bar (LTD calc start/ LTD calc end). If the user selects "I would like this coverage," the form expands the collapsed section. I would like to do the same thing for the previous section ("Voluntary Short-Term Disability Insurance).

Hello,
I just reviewed your form. It seems that you removed the collapse sections. It is possible to have many collapse section in a form. I have created a small sample for you to review and clone if you like.

I am sorry. I still don't see it working.
It looks like I have corrupted your form, but please see my original http://form.jotform.us/form/52426685038157 and do a preview. Scroll down to the "Voluntary Long-term Disability Insurance" section and click on "I would like this coverage." The section that was collapsed (with the calculations) opens up. That is what I want to happen. And I want to same thing to occur in the previous section, like this:

Hi,
Instead of using a collapse field you can also use the show/hide condition.
I have clone your form and added this condition. Since the field does not have any label, I placed my name in the field.

For some reason, I am having trouble replicating this in another section.

In the Flexible Spending Accounts/HealthCare FSA section (towards the beginning of the form), the "Per-pay healthcare amount" field shows, when I want it to show only if "I would like to participate" is selected. The other fields are hidden - it's just the "per-pay" field that I can't make hidden.

Hello,
I was just reviewing your form. This condition indicates that if the radio box equals "I would like to participate", it will show 2 fields. If you like to hide the Per-Pay Healthcare FSA field, just remove it from this condition.

Maybe I should clarify better...
I don't want the "Per-pay healthcare amount" field to show unless the "I would like to participate" button is selected. Currently, it is appearing before the button is selected.
See the condition for "Per-pay healthcare amount" - I am not sure if it is causing the problem, but I would like to keep that condition: If the amount is greater than 2550, then hide the "Per-pay healthcare amount" and show the "Maximum HealthCare Election" field -as it is working now.

If I understood you correctly you want that field "Per-pay healthcare amount" is only shown if "I would like to participate" button is selected.
You can achieve that by setting this condition.
