Why Does the Downloaded Form NOT LOOK LIKE the Form Created?

    When I create a Form (and download) a form after editing the form online, it does not look like the form I created. 

    For example, the form I create has the Label to the "Left" of the Text Box. However, when the form is created and downloaded, the Label is "Above" the Text Box. 

    As a result, my 2-Page form becomes 3 pages when the form is created and downloaded.

    One attached form is correct... and the other is not. So, how do I fix this? What caused a previous form to download correctly, and a new edited form to download incorrect?

    To my understanding you are wondering how come your downloaded form doesn't display the same exact way as the online version of your form.

    I see you appear to have three similar forms on your account but two of them seem to resemble your above screenshot more closely though I'm unsure which.

    The create-a-pdf option is fairly limited in how it will display when downloading a copy of your form whereas if you want something that more closely resembles your form then our fillable pdf creator would be much better for this but the fields will vary slightly.
