We are creating a survey for employees and would like to know who completed the form but do not want to include name or email address as survey fields. The link to the survey will be included in an email. Is there any way to capture that email address when the user opens the form?

I am not sure if I have understood your question correctly.
Do you mean to say that you want to capture the email address of your user but do not want to add an email field in your form?
Please be noted that unless you have an email field in your form, it is not possible to capture the email address of the form's submitter.

I suspected that would be the answer. We want to allow the responses to be anonymous, but I believe that means the only identifying information we can capture is the IP address. Is that correct?
