I have two forms:
1.) http://www.jotform.ca/form/51663954209259
and
2.)http://www.jotform.ca/form/51525683165256
When you hit submit on the first form it redirects you to the second form. Both are setup so that once a user hits submit, an email notification is sent to the SAME email address. However, only the first form is sending a notification email but the second form is not!
The last time the second form sent a notification email was on August 26th, I have already tried several things such as whitelisting "noreply@jotform.com" on my 1and1 hosting account.
Why does the first form send an email but not the second one?

This form http://form.jotform.ca/form/51663954209259 shows an empty email notification, please rebuild it.
The second form email notification seems to be fine, it sends all the fields to officemanager@naturomedic.com
I would recommend you to whitelist our domains in the @naturomedic.com server
