I work for Sea Mar Communiuty Health Centers and an ex employee had build us a form in your website but I cannot acces the submissions

I need the login information of Sea Mar to see how many submissions we have for our event which is 9/19. Here is the link to the actual form. http://www.jotformpro.com/form/51824746016960 but I need to access how many submissions we have and that can only be done with the login information that I don't have.
