It seems all the 3 desired usernames you have shared is same. Please be noted that JotForm username is not case sensitive.
Unfortunately username "MichaelJohn" is already taken. Please let us know any other desired username and if it is still available, we will change your account username. Please be noted that you can yourself check if any username is still available by going to "Sign Up" page. Please check the screenshot below:
Please check the username availability but do not create the account.
