    We are very strict when it comes to login forms. The Phishing filter in place will always suspend the form and accounts once it detect any type of login mechanism from the form.

    I checked your lone form http://www.jotform.me/form/52341936059458 on your account, but so far I cannot understand yet why you need a login feature to this form. If you can provide more details of what you wanted to achieve, we will check for further option and suggestion.

    I also suggest checking the Encrypted Form feature of JotForm.

    Perhaps using encrypted form is the security measure you were looking for.

    Thanks for replying. Basically I am engineering student and i m running an educational  group  .Through this form i can collect information by uploading file through only selected access  .Other wise any body can upload unwanted file

    Alternatively you can embed the form on a website with a pasword protected access to the form, this way users will be required to provide login information before accessing the page where the form is embedded in.
