Form Disabled?

Hi am not sure why all my forms are disabled. My username is:

femininefitnessforce

The Phishing filter had auto suspended your account and forms due to the suspected phishing form http://www.jotform.ca/form/52474276286262 . The form contains a password field which the phishing filter detect as an attempt to use a login form.

But I have manually reviewed the form and I found that it is not a phishing form.
