Here's the JotForm link that has problem - http://form.jotformpro.com/form/52264466817967

I need an urgent response, I was unable to submit the form after completing all the details & uploading the files. It was working fine yesterday but suddenly this morning we weren't able to submit it.

Our event is next weekend thus we need your help as soon as possible.

The issue seems to be caused by Matrix Dynamique widget. Please do consider using one widget instead of 3 different ones, by setting the Column Headers field with all 3 inputs, which would result in a string like this one:

    Column Headers | Full Name:170,Email:170,Phone:170

Once you do it would load one widget, so the loading would be done faster and name, email addres and phone numbers would be associated with the same person (row per person).

At this time it seems that it is having a bit of issue loading everything on your form including the 3 widgets one after another with this single entry.

Do let us know how it goes.

Thank you for your feedback. Nope, it is still not working.. Here's the link http://form.jotformpro.com/form/52493111110944

Also, the problem with that option is that it is not mobile friendly. When you tried to open on your mobile, it will look mess up.

    Please help!!!!!

I tested your form here and it is working fine. My test submission went through.

Base on the screenshot you have posted above, it seems the multiple file upload field on your form appears to be a native single file upload field so I suspect it was a rendering problem. The form is not yet fully loaded on the browser that's why some features are not working properly when you performed a test. On the initial load of the form, the 2 large images are definitely affecting the loading process so I would suggest to resize those images as much as possible to reduce its file size so the form would load faster. Then please clear your browsers cache before testing the form again. This should help.

    Hello ctamyna,

I did check your form #52264466817967 and it does not seems to have any YouTube widget. Have you deleted them already? Is your form working as expected now?

I cloned your form and added a your tube widget. I was able to replicate the issue you are having. I was not able to submit the form as the submit button just says "Please wait.."

The issue seems to be the youTube widget is added within the to form collapse field. If you move the youtube widget before the first form collapse or in the last form collapse, your form will work as expected.

Let me report this issue to our backend team. We will get back to you as soon as we have any update form them.

    Thank you!

    Hello,

The issue is not related to form collapse field. It seem is the form has YouTube widget, the form cannot be submitted.

I have reported this issue to our back end team. We will get back to you as soon as this issue is fixed.

    Thank you!

I should have responded with a little more detail in my initial response - apologies.

    ~

Yes and No - the issue was caused by the Widget Type (The different types of widgets are truly relevant in our development process).

So yes, it was caused by the YouTube widget (and would have been caused by any other widget of a similar type)

So the problem was high level one - a server error - which was actually resolved by our DevOps team.

Sorry for the trouble this caused you.
