Form Not Available

I have created two forms already to serve one purpose but neither of them are working. The one I would like to use named FOMA Registration. I want it to be a secure form also.

Now, my account has been suspended for phishing??? I dont know what that is. Help, please. I need my forms...

I would like to apologize for the inconvenience this have caused you. I have checked your account and it was suspected of phishing by our Anti-Phishing system that is why you got your account auto-suspended. I have checked the forms on your account and found a field called "Username" on one of your forms, the system might have suspected that the form is collecting login credentials. The form seems good so I have whitelisted it and activated your account. This might still have issues in the future and if it is possible for you to change that to something else, that would be great.
