Not a pro at this editing stuff. I went in successfully to add my 4th option under PayPal Form and somehow the form was deleted. Can you help

Uder PayPal  form I added a 4th option - class $35 USD, somehow it was deleted or disappeared. My web designer is on vacation and so I made this a mess all by myself. 

It is possible that my PayPal submission is floating around in my forms somewhere. I need help ASAP

I was able to see the options on the form embedded on your web page, I was able to see a 4th option, but it is not for $35.00 I suppose.
