Someone trying to upload photos gets "Webpage is not available"

We have someone trying to fill out the form linked below and they keep getting an error page every time they try to upload an image through the "Action Photo" upload. 

I checked everywhere online for maximum file size attachments and the only thing I could find was each uploaded file could be up to 1 GB and each form was limited to 25 total uploads. I didn't see anything about the total maximum limit for a form so I don't know if that could be the issue or not since 25 GBs per submission seems like a huge number.

Any assistance you can provide would be greatly appreciated.

I've tried submitting your JotForm and I was able to submit it normally without any issue after uploading the images.
